Community Habilitation Services
We offer a specialized Medicaid-funded program through the Office for People with Developmental Disabilities (OPWDD) that supports individuals with developmental disabilities in gaining greater independence and actively participating in their communities.
Our services are available in a person’s own home or in various community settings, ensuring flexibility and convenience. Our dedicated professional team collaborates one-on-one with each person and their family to design personalized schedules of activities that align with the individual’s unique interests, goals, and preferences.
Through this person-centered approach, we empower individuals to thrive, engage, and build connections within their communities.
